Payments & Policies Client Role Your insurance policy is a contract between you and your insurance company. It is your responsibility to:
*Understand your personal policy benefits, deductibles and co-payments. *Have your current insurance card and referral at the time of your first appointment.
*Notify your practitioner if you have any change in your insurance coverage or provider.
*Fill out our registration form completely for our records and ease of billing.
*Maintain current address and phone numbers with our office.
*Make immediate contact with your insurance company when an EOB (Explanation of Benefits) does not make payment to the practitioner.
*Make payments to practitioner for all co-pays and sessions that your insurance company does not pay or cover to avoid interest and penalties. Practitioner Role
*Notify insurance company with appropriate billing form to inform insurance company that their client has been to practitioner for a session.
*Maintain records.
*Send records as requested by physicians, attorneys and insurance companies.
